State-authorized Supported Community Connections (SCC) for adults with intellectual and developmental disabilities. Paid for by Colorado's Medicaid DD Waiver, with door-to-door rides for eligible clients.

Who it's for
Adults 18 and older with an intellectual or developmental disability who are on Colorado's DD Waiver. We keep the group small so everyone gets real one-to-one and small-group attention. That matters a lot for adults with autism, complex communication or higher support needs. ElizaBeth has years of experience with people who use few or no words.
How it's paid for
The program is funded through the Medicaid DD Waiver as Supported Community Connections (SCC). It is not private pay. RMHS families can request extra Mill Levy SCC hours on top of their waiver hours. We'll also show you how much the state pays and how much we keep, so you know your loved one's funding goes to their care.

Every plan is built around what the person enjoys and the goals they're working on.
- Weaving and fiber arts
- Music, drums and creative play
- Cooking meals together
- Time outside every day: parks, walks, gardening
- Fine and gross motor work through games and movement
- Independence and pre-employment skills

A short call
Send the form or call. ElizaBeth sets up a 20 to 30 minute call to learn about your loved one, their interests and their support needs.

Come visit
See the space near Monaco and Colfax, meet the team and watch a normal day. Bring your loved one if you can.

We handle the paperwork
We work with your case manager at RMHS, Developmental Pathways or your agency to confirm SCC on the DD Waiver, then pick a start date that works for you.

How is it paid for?+
Through Colorado's Medicaid Developmental Disabilities (DD) Waiver, as Supported Community Connections (SCC). It is not private pay. If your loved one is served by Rocky Mountain Human Services (RMHS), you can request extra Mill Levy SCC hours on top of the waiver hours.
Which case management agencies do you work with?+
We are contracted with Rocky Mountain Human Services (RMHS) and Developmental Pathways, and we have room to work with other case management agencies.
Do you offer rides?+
Yes. Non-Medical Transport (NMT) gives eligible clients door-to-door rides to and from the program.
What are the hours?+
Monday through Thursday, 10am to 4pm.
Where are you?+
Near Monaco and Colfax in Denver. Families come from Park Hill, Lowry, Aurora, Central Park, Cherry Creek and nearby neighborhoods.
