Medicaid-authorized care, built around the person.

State-authorized SCC on the Medicaid DD Waiver, extra Mill Levy SCC for RMHS families, Non-Medical Transport, and residential services. We work to keep more of your loved one's funding on their actual care, not on agency overhead.

Supported Community Connections (SCC)

We're authorized by the State of Colorado to provide SCC. Most SCC is paid for by Medicaid through the DD Waiver. We're also a Mill Levy provider for Rocky Mountain Human Services (RMHS), so families can request extra Mill Levy SCC hours on top of their Medicaid hours. We work with both RMHS and Developmental Pathways as Case Management Agencies (CMAs). Clients pick up crafts that fit what they enjoy, fiber arts, home decorations, music, cooking, and work on fine and gross motor skills through exercise, movement, and time outside every day.

Non-Medical Transport (NMT)

Reliable, friendly rides to and from our program. We handle the driving, so families get a little of their day back and clients don't miss a day because nobody could give them a ride.

Residential Services

We've been providing residential support for adults with I/DD for over a year and a half. The care providers are who make this work, so we pay them well above what a lot of agencies pay and we listen to what they tell us. We also keep agency overhead low. Most agencies won't show families the state HCBS reimbursement sheet, so nobody knows how much of the funding the agency is keeping. We'll show you ours, and we keep more of it on the client.

Frequently asked questions

Who is eligible for your day program?+

Adults with intellectual or developmental disabilities (I/DD). Eligibility depends on having the Developmental Disabilities (DD) Waiver through Medicaid, these services are funded through the DD Waiver, not privately paid for. Private pay is out of reach for nearly all families. ElizaBeth has extensive experience supporting individuals with minimal verbal language.

What kind of SCC do you provide?+

We are authorized by the State of Colorado to provide Supported Community Connections (SCC). The vast majority of SCC is paid for by Medicaid through the DD Waiver. We are also a Mill Levy provider for Rocky Mountain Human Services (RMHS), so parents and caregivers can request additional Mill Levy-funded SCC hours on top of their Medicaid SCC, but most SCC is not funded through Mill Levy.

How transparent are you about pricing?+

Rates aren't something providers set, they're determined by the state. What we are transparent about is how much the state pays and how much we retain. Most agencies don't share the HCBS reimbursement sheet with parents, so families have no idea how much of their loved one's funding goes to agency overhead. We do.
